目的 探讨老年2型糖尿病(T2DM)伴发精神障碍的抗精神病治疗的效果.方法 将60例老年T2DM伴发精神障碍患者分成抗精神病组和对照组(每组30例),进行12周的对照研究.两组均釆用相同的内科治疗,抗精神病组采用小剂量的富马酸喹硫平治疗和心理治疗相结合.治疗前后测定FBG、2hPBG、HbA1c、及使用简明精神病量表(BPRS)评定治疗的效果.结果 抗精神病组BPRS分明显低于对照组.抗精神病治疗后FBG、2hPBG 、HbA1c明显降低,与对照组比较均有统计学差异(均P＜0.05).结论 联用小剂量喹硫平治疗老年T2DM伴发精神障碍,无严重的不良反应,耐受性好,依从性高,具有良好的临床疗效.%Objective To evaluate the effectiveness of antipsychotic therapy in elderly type 2 diabetic (T2DM)patients complicated with mental disorders. Methods Sixty elderly T2DM patients complicated with mental disorders were divided into antipsychotic group (n=30) and control group (n=30). Both groups were given the routine medical treatment for 12 weeks; small dose quetiapine fumarate and psychological therapy were additionally give to antipsychotic group. The therapeutic effectiveness was evaluated with Brief Psychiatric Rating Scale (BPRS) and FBG, 2hPBG and HbAlc were determined before and after treatment in both groups. Results The BPRS scores in antipsychotic group were significantly lower than those in control group. The FBG, 2hPBG and HbAlc decreased significantly after antipsychotic treatment (P<0.05) compared with control group. Conclusion Small dose quetiapine and psychological therapy combined with routine medication are effective for elderly T2DM complicated with mental disorders with well tolerability, compliance and no serious adverse events.
